Het herfstweer slaat toe en onder een warm deken zit Esther (54) op de bank klaar voor het interview. Ze heeft een goede week, wat betekent dat ze energie heeft. In de middag gaat de stad in met een vriendin, ze wil een nieuwe pyjama.
Esther kreeg nierbekkenkanker door roken: 'Trein met slechts één sombere eindbestemming'
Het zijn de kleine dingen waar ze van geniet nu het nog kan. Esther heeft namelijk te horen gekregen dat ze niet meer zal genezen van nierbekkenkanker.
Tabakswaren
Vorig jaar december werd Esther gediagnosticeerd met een zeldzame, agressieve vorm van kanker die veroorzaakt wordt door het inademen van de schadelijke, kankerverwekkende stoffen die in tabakswaren zitten. Na veertig jaar roken is ze op dringend advies van haar arts onmiddellijk gestopt. Iedere sigaret zou de kans op uitbreiding van de ziekte vergroten.
“Er gebeurt ontzettend veel als je gediagnosticeerd wordt. Je leven staat op de kop. Opeens bekijk je het leven door een andere bril.” Esthers rechternier en urineleider zijn begin dit jaar verwijderd en in eerste instantie was er goede hoop. Ze kreeg een positieve uitslag en een nieuwe controlescan zou pas over enkele maanden nodig zijn.
Chemotherapie
In juni voelde Esther zich niet goed: “Ik voelde van alles, ik had pijntjes en narigheden.” De onrust werd steeds groter dus trok ze aan de bel. “Ik las medische rapporten en besefte dat als het mis ging, het goed mis zou gaan.” De controlescan werd vervroegd en wat bleek: de kanker had zich verspreid in haar buikholte en buikvlies. “Mijn gezin en ik kregen te horen dat ik niet meer zal genezen.” De prognose is somber, maar omdat ze jong is, wordt er nog van alles uit de kast getrokken en ingezet op kwaliteit van leven.
“Ik werd verrast door mijn oncoloog die chemotherapie aanbood. Hij vertelde dat ik een reële kans maakte op het pauzeren van de groei van de kankercellen en het verbeteren van de kwaliteit van leven en zelfs levensverlenging. Maar de uitkomst blijft hetzelfde. We zijn er kapot van. Aan de andere kant moet ik de koe bij de horens pakken. Ik weet niet hoe lang ik nog heb en daar moet mijn gezin en ik mee leren leven.”

De meeste mensen zijn kennen borst-, mond-, keel- en longkanker als gevolgen van roken. Nierbekkenkanker is minder bekend, en die van Esther is meedogenloos en genadeloos. “Deze vorm is dikke pech en heeft slechts één eindbestemming. Ik vroeg de arts hoe lang ik nog te leven heb, maar daar wordt niet over gesproken. Dat blijft lastig om te accepteren in combinatie met de wetenschap dat ik mijn leven verlies door het roken. Er is geen andere waarheid en daar moet ik met dealen.”
Voor Esther heeft kanker een metgezel gekregen en dat is spijt. “Wat ik nu nog kan doen is van de daken schreeuwen dat ik kom te overlijden aan de gevolgen van roken, in de hoop iemand te inspireren om te stoppen met roken. Als ik één iemand kan overtuigen dat je de kracht hebt om te stoppen, dan ben ik tevreden. Zo was er bijvoorbeeld een vader die mijn verhaal besprak met zijn zoons. Dat maakte indruk op hen. Een gezicht geven aan een verhaal over de gevolgen van roken is zinvol. Dan is het echt. Ook een lerares werd geraakt door mijn verhaal en gaat mijn pechverhaal als onderdeel van lesmateriaal inzetten.”
Spijt
Esther voelt zich schuldig, en dat gevoel wordt gevoed door spijt. “Ik kan er niet omheen, het is onomkeerbaar. Het is de naakte waarheid dat ik mijn leven verlies door roken. Ik gun het niemand.” Ze is trots op alle mensen die stoppen met roken en alle initiatieven die voorbij komen. “Jarenlang hoor je als roker dat je er kanker van kunt krijgen, maar dat het moeilijk is om te stoppen. Woorden als ‘moeilijk’ worden telkens herhaald.”
Als het aan Esther ligt, moeten we de nadruk leggen op het feit dat stoppen wel degelijk mogelijk is. “Als we zinnen als ‘jij kunt het ook’ herhalen, geef je de roker kracht om te stoppen en zal diegene zich gesteund voelen. Het is niet eenvoudig, maar als je constant om je heen hoort dat onmogelijk is, is dat niet bevorderend voor je motivatie.” En daar moet verandering in komen, vindt Esther. “Ga een gesprek aan met je huisarts en onderzoek de mogelijkheden om te stoppen. Met een plan de campagne is het slagingspercentage groter.”
Sombere eindbestemming
Esther bewandelt een nieuw pad waarin ze probeert te navigeren in een land dat ‘kanker’ heet. Artsen proberen antwoorden te geven, familie en vrienden bieden steun, maar onderaan de streep maak je deze reis alleen. “Je wil de naakte waarheid niet meteen op tafel gooien, maar het moment is daar: ik zit in een trein met slechts één sombere eindbestemming. Dierbaren vragen mij welke dingen ik nog belangrijk vind om te doen, en welke juist helemaal niet. Deze gesprekken zijn heel waardevol. Een vriendin zorgt voor mensen die in de terminale fase zijn beland en ik belde haar als eerst met de vraag of ze ook voor mij zou willen zorgen aan huis. ‘Dan zie ik je tenminste nog eens’, grapte ik.”
Esthers familie en vrienden doen veel voor haar, maar iets terugdoen gaat nauwelijks. Dat vindt ze lastig. “Het is eenrichtingsverkeer en dat doet pijn.” Stiekem geniet Esther daar ook van, want nu ziet ze haar broer, zussen en vriendinnen vaker dan ooit. “Ik waak ervoor dat ik geen kankerpatiënt ben, maar een mens met de diagnose kanker. Ik geniet van kleine de momenten, zoals samen met een vriendin lunchen in de stad en shoppen voor een nieuwe pyjama. Ik koester de kleine geluksmomenten.”
